The forms located HERE have been approved for use by the Illinois Supreme Court Commission on Access to Justice and are required to be accepted in all Illinois courts. There are printable forms for many different types of cases.

Federal and state tax forms are now available online. You may access and print these at home, or from one of the library's computers for a fee of 20 cents per page. We do have a small selection of tax forms and instruction booklets available on a first-come, first-serve basis.

The primary responsibility of the Department of Revenue is to serve as the tax collection agency for state government and for local governments. The department also administers the state’s lottery and regulates the manufacture, distribution, and sale of alcoholic beverages. The department also oversees local property tax assessments, and functions as the funding agent for the Illinois Housing Development Authority.
